Your Rights as a Client

As a client of Catholic Charities North Dakota, you have the right to:
 
  • Be treated with dignity, respect, and fairness
  • Receive services without discrimination
  • Be informed about the services available to you
  • Participate in developing your service plan when applicable
  • Ask questions and receive clear explanations
  • Have your personal and health information kept confidential in accordance with HIPAA and other applicable laws
  • Review and request access to your records as permitted by law
  • Express concerns or file a grievance without fear of retaliation
  • Receive services in a safe and professional environment

Your Responsibilities as a Client

To help us provide the best possible care and support, clients are expected to:

  • Provide accurate and complete information
  • Participate actively in services and follow agreed upon plans
  • Treat staff and other clients with respect
  • Keep scheduled appointments or notify us in advance if you need to cancel
  • Follow program guidelines and policies
  • Ask questions if you do not understand something
